Important before owners begin the process of TOPA owners must email DOH directly to DOH.TenantPurchase@cityofchicago.org 

Subject Line: “Notice of Intent to Sell [Property Address] (606 District) 

• Owner's Intent to Sell Form  - Prior to listing the property, the home owner is required to give every tenant in the building an Owner's Intent to Sell form. As a way to notify their tenants that the building is going up for sale.
  

• Owner Declaration of Intent to Sell - Once the home owner intends to sell their property it is necessary for the owner to fill out the Owner Declaration of Intent to Sell form and submit it to the DoH (Department of Housing) via their E-Form Portal 
  

• Notice of Third-Party Purchase Agreement - Once you have accepted an offer from a Third-Party Purchaser you must provide tenants with a Notice of Third-Party Purchase Agreement form
